where to find a location

I live in Lyons, Illinois, to be exact, and I am absolutely looking for a clinic, the b12-LiPo-shot program. I was on something similar, where I would get three images per week and 14 shakes for a week, for a total of 700 calories per day and it worked well for me, it was Weigh Inn in Chicago and the clinic nurse who led the deaths on a decade ago.
